| SERVICES | The following is a brief synopsis of some of the services that Safe Haven Quality Care, LLC offers. |

| | | | | Personal Care: Duties are typically performed by a Certified Nursing Assistant. Helps clients with bathing, dressing and grooming. Prepares and serves food for clients following special dietary requirements. Reminds clients to take prescribed medications as directed by physician. Provides socialization for clients and maintains records of service provided and client’s condition, reporting any change in condition to the Agency staff, Case Manager and the family. Changes bed linens, maintains the client’s laundry, and performs light cleaning of the client’s living quarters. Accompanies ambulatory clients outside the home. Attends monthly mandatory in-service training. | | TOP | | | Homemaking: Duties may be provided by a Home Health Aide who is responsible for providing personal care services according to written directions. To facilitate a client remaining in their own home, these duties may include accompanying clients to their appointments, providing clients with a clean and safe living environment, meal planning and preparation, shopping and light housekeeping such as mopping, vacuuming, dusting and sweeping. Advises Safe Haven staff of substantial changes in the client’s health or well-being. Attends monthly mandatory in-service training. | | TOP | | | Respite and Companionship: Duties are designed to give clients and caregivers non-medical support and relief from 24-hour care routines by providing constant supervision of clients. Personal Care, Homemaking and socialization (games, reading, listening) are applicable as well as safety and home accident prevention. Attends monthly mandatory in-service training. |
| Skilled Nursing Services: 1. Supervision. Safe Haven Skilled Services, LLC shall provide skilled nursing services by and under the supervision of a registered nurse in accordance with the plan of treatment. 2. Duties of the Registered Nurse. The registered nurse shall make the initial evaluation visit, regularly reevaluate the patient’s nursing needs, initiate the plan of treatment and necessary revisions, provide services requiring substantial specialized nursing skills, initiate appropriate preventive and rehabilitative nursing procedures, prepare clinical and progress notes, coordinate services, inform the physician and other personnel of changes in the patient’s condition and needs, counsel the patient and family in meeting nursing and related needs, participate in in-service programs and supervise and teach other nursing personnel. 3. Duties of the Licensed Practical Nurse. The Licensed Practical Nurse shall provide services in accordance with agency policies, prepare clinical and progress notes, assist the physician or registered nurse in performing specialized procedures, prepare equipment and materials for treatments, observing aseptic technique as required and assist the patient in learning appropriate self-care techniques.
